If you are not able to see the Surveys section, it is likely because your study is currently categorized as "Retrospective." Surveys are strictly a feature for "Prospective", "Registry" or "Other" study designs:
To enable the Surveys tab:
Navigate to your study and open the Settings section, then click on Study.
Select Study Properties.
Locate the "Study Design" field. You must change this from "Retrospective" to either Prospective, Registry or Other.
Once the design type is updated, scroll to the bottom of the page. You will now see the option to enable Surveys module.
